Wellness Series 3 – Closing the Gap with Diabetes

In part 1 of this series, we introduced the 12-year gap between how long we live and how long we live well. Diabetes is one of the biggest contributors to that gap; however, it is also one of the most manageable. The difference between well-controlled and poorly controlled diabetes is measured in years of vision, kidney function, nerve health, and heart health.

What actually shrinks the gap?

Knowing your numbers in real time. Time in range is the modern scoreboard. A continuous glucose monitor shows you not just where your glucose is, but where it is heading and what moved it. Patients who see the effect of a meal or a walk in real time make better decisions without willpower battles. If you are still using finger sticks and qualify for a CGM, ask us. Coverage has expanded significantly.

Aim for time in range, not just your A1c. A1c is a three-month average. Two people with the same A1c can have very different days, one steady and one swinging between highs and lows. The swings do the damage. Most adults should target 70 percent or more of the day between 70 and 180 mg/dL. Your care team can set your personal targets.

Walk after meals. Ten to fifteen minutes of walking after eating blunts the glucose spike. It is the cheapest diabetes medication that exists. Your CGM will show you the effect the first time you try it.

Build meals around protein and fiber first. Eating protein and vegetables before starches flattens the post-meal curve. You do not need a perfect diet. You need a repeatable one.

Protect your feet and your eyes. Annual dilated eye exams and daily foot checks catch problems while they are still fixable. Neuropathy and retinopathy announce themselves late. Screening finds them early.

Do not skip the boring appointments. Kidney function labs, blood pressure checks, and statin conversations are where health span is defended. Bring your CGM data. It makes every appointment more productive.

What is the payoff?

Well-managed diabetes is compatible with a long, fully functional life. The tools have never been better, and the data has never been easier to see.
